Our practice participates in majority of the local insurance plans as well as Medicare and Medicaid.  We will need to make copies of all of your insurance cards for your chart so please bring them to every office visit.  If you change your address, phone number, primary care physician or insurance coverage, please notify our office staff as soon as possible.

If you are a member of an HMO or PPO plan, your plan may require a referral from your primary care physician, please have one issued by your primary care physician and bring it at the time of the visit.

For all office-based services deductible and/or your copayment are due at the time of service.  We accept cash, check, debit and credit card payments.

Financial hardship situations are considered on an individual basis, please discuss your situation with the office manager to explore a solution for an affordable payment plan.
